Question : Hello, doctor. I have a question about bladder meds and which, if any, would be of help to me. I do not have incontinence - actually just the opposite. Bad urge to go (a feeling I get about every 10 min) but nothing in the bladder. Get a tiny amount out. Feels like bladder is inflamed and I also have nerve issues for many years so may be neurological factor as well. Over the years, have been prescribed oxybutin, vesicare, and the new one, Gemtesa. I believe my doctor said Gemtesa may have nerve relaxing component. In any event, would like your opinion on these meds and any other suggestions u can give. Thank u so much....

Brief Answer:
You are likely to have what is called as Senile Bladder Neck Stenosis.

Detailed Answer:
Hello
Thanks for your query
Based on the facts that you have posted and that you have been taking Gemtesa since long time with unsatisfactory results I think your problem is due to what is called as Senile Urethral Stenosis.
which is very common in females at this age .

As the age advances especially after menopause all the Genito Urinary organs get atrophied causing narrowing of urethra. This narrowing of Urethra leads to incomplete emptying of bladder during micturition and irritation of bladder causing urge to pass urine .

I think Endoscopic evaluation of bladder and Calibration of Urethra will help to resolve your symptoms.

Discuss this with your Urologist and get it done .

It is a very simple procedure done as an out door patient and does not need hospitalisation .

In the meantime continue taking Gemtesa once daily .

Thanks and Regards
Dr Patil
