Question : My mother, Ms. XXXXXXX is 61 years old. She was first diagnosed with a gallstone in March 2021 (9 mm). Follow-up ultrasounds showed progression: 14 mm in March 2022, multiple stones with the largest 11 mm in August 2024, and now a 13 mm stone in July 2025. She remains asymptomatic.
She is a pure vegetarian, avoids fried foods, and walks 7,500 steps daily in the evening. She follows intermittent fasting with two meals a day — brunch at 12:30 PM and dinner between 7:30–8:00 PM.
Her diet includes ragi mudde, red rice, rotti (jowar or bajra), chapathi, and dosa — all prepared at home with care. However, she has lost all molars in both jaws, which slows chewing and limits intake of coarse or raw foods.
She has prediabetes (HbA1c 6.1%), is on Syrovas 5 mg for cholesterol, and has mild insulin resistance (HOMA-IR = 2.6).
Given this 4+ year history of gallstone progression and recurrence, her metabolic risk factors (prediabetes, dyslipidemia, insulin resistance), and chewing-related digestive challenges, would you recommend elective laparoscopic gallbladder removal now — before symptoms or complications arise?
