Question: follow up query for Dr XXXXXXX ,38 year old male. I am writing in follow up to a question from last week. Had RIGHT sided dull scrotal pain in December 2023. Have dull pain above right pelvic spine area. Urologist performed manual exam and felt that both testicles felt normal. Ultrasound was performed in December 2023 and was normal as to the testicles. However, two findings were reported related to the epididymides: (LEFT) Left epididymis: Normal size and vascularity. 9 mm echogenic focus in the left epididymal head, possibly a small lipomatous lesion. Left hydrocele: None. (RIGHT) Right epididymis: Normal aside from a 5 mm cyst with rim calcification in the body. Right hydrocele: None. Right varicocele: None. (<.2.5mm) Ultrasound was repeated last week. Testes were normal. These are the findings as to epididymides: (4/1/2023) Epididymal heads: Right measures 1.2 x 1.1 x 1.1 cm. No discrete abnormality evident. Probable appendix testis noted measuring 7 x 5 x 3 mm. This was evident on 2016 examination is unchanged. On the December 26, 2023 exam, a the small nodular echogenic was present within the body habitus. This region is not well visualized. Left epididymal head measures 1.3 x 0.9 x 1.1 cm and contains a 6 × 5 × 4 mm nodular echogenic nonshadowing area. On the prior/December 26, 2023 exam this area measured 5 x 4 x 9 mm. This was not evident 2016. Appearance is nonspecific. Fat-containing lesion is a possibility. Some small granuloma could give this appearance. Complex spermatocele is also consideration. Epidermoid cyst less likely. More aggressive pathology less likely but difficult to exclude. Follow-up testicular ultrasound in 3-6 months suggested. My questions are as follows: 1) I seem to read that epididymal cysts occur mainly in the head and not the body. Could this still be a cyst with calcification in the body of the right epididymis? 2) Is rim calcification a common finding here? I can see it on the ultrasound image. 3) As to the LEFT epididymis, I have developed a concern that this is a sarcoma due to the description. Is it a good sign that it hasn’t changed size month in the three months? Is there anything else reassuring about this ultrasound that I can take my mind off of epididymal or spermatic cord cancer? 4) After having years of masturbation addiction, I stopped somewhat abruptly in September, and have probably only ejaculated 6 or 7 times or so since September. Could either have significance here?

In my 50 years of practice I have not seen a patient of cancer of spermatic cord or Epididymis.

There is no report of Radiologist who did Ultrasound Scanning attached to your query .But from your statements that you have described it can be a soft tissue benign tumour.

Repeat your Ultrasound scanning and upload report.
