Brief Answer:
strained muscle or muscle spasm due to overuse or repetitive movements

Detailed Answer:
Hello,

I have gone through your query and here is my advice.

As per your history, all investigations are normal, so it may be strained muscle or muscle spasm due to repetitive movements or overuse.

Still I would suggest you to consult with spine surgeon for detailed clinical examination. He may procure nerve conduction studies as per evaluation.

Meanwhile, continue physical therapy. You can take muscle relaxant chlorzoxazone and pain medications tablet motrin after family physician consultation. Apply diclofenac gel locally. Gentle application of heating pads will also helps.

Along with that avoid heavy weight lifting, forward bending and squatting position. Keep yourself hydrated.

Hope I have answered your query.
Let me know if I can assist you further.
