Question: hello doctor,

my mother age 48 is suffering from back pain (not frequently)she gets weak and get fever during this pain.
pleaee suggest how to proceed further?

Back pain arises from wide variety of causes. It is difficult to narrow down on a cause based on the limited information provided.

1. May I know since when is she suffering with back ache?
2. Does the pain also involve other parts of the body such as legs and other joints?
3. Has she been investigated in the past by a doctor? If yes do you have previous medical records?

Since she becomes febrile during the back ache episodes, there are high chances it is related to an inflammatory condition. Therefore it warrants a need for X-rays / MRI scans along with tests for inflammatory markers. I'd suggest visiting a rheumatologist / orthopedic specialist to be evaluated further.

Until then, she can use painkillers such as tab aceclofenac 100mg as and when she has backache. Rest adequately and avoid strenuous physical activities.

I went through the records uploaded. As I see, her doctors have advised for Xray and bone density test, was it performed? If yes, can you upload the results of those or other tests performed?

Has she undergone physiotherapy as recommended and what was the response after physio?

I went through the X-rays attached and as reported the X-rays appear normal and there are no features suggesting bone depletion. Physiotherapy is known to provide good pain relief for many patients with similar back pain and hence it should be followed.

In addition, you may also consider consulting a rheumatologist to evaluate for inflammatory conditions since there is presence of fever with back pain. More tests may be ordered by rheumatologist if necessary to understand the cause and suggest treatment.

If you are only looking for temporary pain relief then there are many pain killers available. Tab Ultracet as suggested by her doctor in the prescription is one very good pain medication; however, you need a recent doctor's prescription to obtain it. Alternatively, other pain killers such as tab aceclofenac MR or etoshine MR tablets can be used for pain relief as and when necessary.
Physiotherapy is a useful option to treat chronic back pains. You should consider it.

However, it would be better to discuss with a rheumatologist or back surgeon to know the cause of back pain. Specific remedies addressing the cause will provide her with long term and effective pain management.
