Hello!

Welcome to Ask a Doctor service!

I understand your concern and would explain that it is quite normal to have higher blood pressure values, ( systolic and diastolic)in sitting and standing up position, compared to lying down.

Coming to this point if these high diastolic blood pressure values persist during the day, they may be indicative of the beginning of arterial hypertension.

For this reason, I would recommend performing some other tests to investigate for possible underlying causes:

- thyroid hormone levels for possible thyroid gland dysfunction
- complete blood count, PCR, ESR for inflammation
- blood electrolytes for possible imbalances
- kidney and liver function tests
- fasting glucose.

You should discuss with your doctor on the above tests . Starting low dose of a calcium channel blocker ( amlodipine lercanidipine, etc) may be needed in order to maintain blood pressure values under control.

Hope you will find this answer helpful!

I remain at your disposal for any further questions whenever you need!

Kind regards,

My doctor ordered all the above tests and all came out to be normal. I am currently taking Diltiazem 180mg Extended Release.

Detailed Answer:
Hello again!

Thank you for the additional information!

I agree with your doctors choice, but you should know that diltiazem acts mainly on the heart rate ( reduce the heart rate) and has a weak effect on high blood pressure.

For this reason, if your diastolic pressure values does not drop, I would recommend switching to amlodipine or lercanidipine.

Hope you will find this answer helpful!

Wishing good health,
