Question: Hello, my 42 year old son has been diagnosed with long Covid for the second time in 2 years. He initially had Covid 9/21, long Covid for 2 months and went back to work after 2 months. Second time with Covid was Sept 2022. He has been diagnosed with Long Covid again but is not improving this time around. He has been out of work and debiltated since Sept 18, 2022. His symptoms have been nausea, vomiting, some diarrhea and chills with occasional fever. Now 3 months later, he still has occasional temps 100-102 ( 3-4 times a week) mainly at night. He has chills and feels cold all the time, SOB on exertion and extreme fatigue sleeping 18 + hours a day. PCP has done labs, HCT was elevated for 1 year since first Covid but recently have returned to normal. He has seen a hematologist who has ruled out any blood diseases. Stool specs have been normal. This has debilitated him completely and he is unable to function ,or work . And PCP has no further ideas except rest and hydrate, he does have sleep apnea and is on CPAP. Lyme and Mono have been ruled out as well. We are desperate and need any advice or what else we should be doing. Thank you for your help. Karen Coffin RN ( concerned mom)

Hello Dr XXXXXXX thank you for your time and help. I have uploaded approx 8 -10 lab results for you , CBC, CMP, stool cultures, UA, 2 TSH levels and test done at hematologists office. I hope you can see them all, if not let me know. XXXXXXX is Hypertensive. I will list meds. He also has sleep apnea, on CPAP. He is not diabetic. No cough lungs are clear per PCP ( and me). No Chest Xrays or EKGs have been done. Thank you for your help. His PCP is a Nurse Practitioner and doing her best but I feel this is complicated, appreciate your advice. 3 months is a long time, they feel it is long Covid again. Thank you. Karen Coffin

CURRENT MED LIST :

-BUPROPION Hcl XL- (Wellbutrin XL) 150 mg tabs- take THREE tabs every morning

-SERTRALINE 100 mg tabs -(Zoloft) Take TWO 100 mg(200 mg) tabs at bedtime

-Potassium Citrate 15 MeQ- 2 tabs twice a day ( help prevent Kidney stone production)morning and suppertime ( do not lie down at least 10 mins after med)

**-METOPROLOL SUCCINATE- 200 mg at bedtime

-NIFEDIPINE ER ( Procardia XL) 90 mg tablet - take one tab at bedtime

-Aspirin enteric 81 mg in AM and PM

Lactobacillus Acidophyllus one tab BID

**-Latuda 40 mg daily at supper time ( new med started 8/18/22 at 20 mg, increased to 40 mg 12/1/22)

I was engaged and couldn't catch up with you.

I have seen all the reports attached here.

With the present symptoms, I consider needing to rule out conditions that can present with intermittent fever. The ones which are common are Ricketssial fever, enteric fever, and brucellosis.

I recommend getting tests for above infection to rule out these, as they can be easily treated.

Thank you Dr I will discuss this with my doctor. I appreciate your help. Last question, I just got my food sensitivity results back, a home test from Everlywell, IGg tests. They said I am Level 2 sensitivity for Milk, and wheat. Level one for casein and many others. Can I take Lactaid for this? Would this cause any of my symptoms? Thank you.

No, your symptoms may not be connected to food sensitivity.

Most of times its gastrointestinal disturbances or hives which you face due to food sensitivity.
