Hi Doctor,  My Wife Is Four Months Into Her Pregnancy

Question:

Hi Doctor,

My wife is four months into her pregnancy. This is my second pregnancy; my first was a C-section in 2018.

LMP 25/08/20202 and cycles of 27 days
35 years old and O-negative

We finished the NT scan at 13 weeks and noticed the baby's nasal bone was not assessed, so the doctor suggested an 18-week early anomaly scan, which we did yesterday (December 29), and then the doctor told her she needed to be on bed rest because they had discovered the placenta previa. Only necessary things were asked to walk.

She is also doing WFH work and has been advised to stop.

Kindly review the attached report; does she need complete bed rest until her next scan? Is it something critical? Can she continue her work from home and her routine life within the house without lifting anything?

Do not travel for more than 10 kilometres at a time (except in exceptional circumstances).
According to the doctor, the placenta can also move upward later in the pregnancy.

But she completed four months, and we were worried a lot during this time. Should she be on complete bed rest?

We accepted that if there is any blood or bleeding, we can reach the hospital within one hour ( this window enough to reach the hospital)?

Kindly review.

Brief Answer:
Need repeat anomaly scan and bed rest.

Detailed Answer:
Hello dear,
Thanks for trusting us about your health care.
I have gone through your history and reports in detail and understand your concerns.
In your report nasal bone is absent ,so it is important to do anomaly scan to rule out any congenital abnormalities.
But in your early anomaly scan ,no obvious congenital abnormalities detected.
In your ultrasound she is having placenta previa ( complete),in this condition bed rest is mandatory to avoid bleeding and misscarriage.
Usually placenta moved up in 28weeks of pregnancy.
So you should not worry much about it, bed rest is important.
And progesterone should be continued.
Avoid much walking and work from home job only in resting position.
She can not travel for 10km.
As such she is not having any problem,so no risk to baby, and not worried much about it.
Once it moves up,no issues.
Hope found useful, feel free to ask more information.
Good luck

Thanks doctor for the information.

To clarify. Bed rest means she should be always sleeping position at bed or she can sit ideally for few hours or continue the work from home in sitting position.

Our doctor haven't prescribed anything
Progesterone except Iron.. Do we need ask them?

Based on ur patients and experience pls suggest.. From our home to city main hospital is one hour travel time to reach.. Is this sufficient window in case of emergency ?

Thank you.

Brief Answer:
Bed rest needed

Detailed Answer:
Hi,
Thanks for your reply.
Sorry for my delay response.
She can do few hours sitting job on bed only( in between gap is important).
Yes,progesterone are usually used to support pregnancy.
It is far from your place because one hour is usually a big distance,it should be 15-20mins.far from your place, because chances of bleeding and premature delivery are there in your case.
You should not worry much about it, everything will be fine once placenta moves upside.
Hope found useful
