Question: Hi Doctor,

Asking for a family friend. He is 38 year old and suffering from AML. He had undergone Chemo and doctors advised to discharge him as his blood counts are normal. But today, they have also said that they would need to do a bone marrow transplant and his sister is identified as donor. Can you let us know why do we have to do transplant, if his blood counts are normal. The bone marrow test is not yet scheduled and the mrd is also not determined yet. Is it mandatory that we have to opt for the transplant as there might be complications like rejection and immunosuppressants have to be taken. Please advise

Thanks for your query.

In AML, some subgroups of patients who have high risk criteria, we recommend consolidation allogeneic BMT after initial chemotherapy. This is because these patients have high risk of disease relapse without transplant.

I am pretty sure they will do the bone marrow to confirm remission status before going for transplant. It can't be said as mandatory for all patients but preferred if he is in any high risk group.

Thank you Doctor for the information. I had informed the same and they are planning to do transplant in a few days. He is discharged now and is able to eat but they have asked to be precautious to avoid infections. Can you please help me with a proper diet information that should be given and what the things that is better to avoid. He already suffered from Intestinal infection during Chemo, so considering that please let us know the food that can increase the risk of infections.

Avoid raw fruits and vegetables

Detailed Answer:
Can take fruits with thick peel like banana. Apart from that can take any home cooked food and maintain proper hygiene in food preparation, storage and handling.
