Thanks for your query.

These reports suggest increase in immunoglobulin due to non malignant cause. Maybe old or recent infection, rheumatic disorders, liver disease, etc. Sometimes no cause is found.

This is not due to multiple myeloma or lymphoma.

Is this a chronic condition or will this go away?

Does this cause any symptoms?

Does liver disease have to be ruled out and how would it be ruled how?

I have read that this can be caused also by non hematologic malignancy, is this correct?

How can other diseases be ruled out with this lab draw here? Inflammatory matkers are negative. Only unconjugated bilirubin is elevated all else is normal.

This is not a disease

Detailed Answer:
By itself, it is not a disease and hence does not require any treatment. It may or may not go away. If otherwise you are fine then not to worry.

If liver function tests normal then liver disease ruled out. Unconj bilirubin elevation is not significant.

Yes this can be due to non hematologic malignancy but if there is any symptom then we can investigate for it. We may not do it just on the basis of this report.

So basically, everything depends on your symptoms. If they are worrisome, we should investigate further.

One last question, say said patient just has weight loss and no other symptoms. How would you investigate for non hematologic malignancy in this patient?

Would patient need to see several doctors to do investigation of malignancy?

Depends on the degree of weight loss

Detailed Answer:
If there's significant weight loss, like 10% in 6months, then we should investigate.

Usually we do PET CT whole body, upper GI endoscopy and colonoscopy for screening.

An internal medicine specialist can do all the investigations.
