Question: hi. i often get headaches late in the day... they start in the back of my head and are really painful. massaging shoulders and neck radiates the pain up through my ear and to my temples.

once i rest, the headache moves to my temples, around the eye... usually more on one side, but the tension is always worse on one side as well.

does this happen with tension headaches? why does neck pain radiate to the sides of my head? it also seems like they dont go away until i fall asleep.

do you think it could be migraine?

I have noted your clinical details. Based on this, features are consistent with a diagnosis of tension headache. Muscle and nerve supply of back of head, temples and upper neck muscles are same, that is the reason pain radiates from neck to sides of head.

The classical features of migraine such as nausea/vomiting/increased sensitivity to light and sound are lacking in your case, hence, migraine is less likely.
