Question: mom 80. was in icu for 3 days. sugers at 750. doing better. hospital doctors put her on insulin. 10 units of lantus in morning. use sliding scale at meal times. suger levels stayed at 250 to 325. I change amounts. give her 29 units of lantus in morning, 3 time a day of nvaloc at 6 units each before meals times. suger numbers now at 150 to 200. Im I giving my mom too much insulin. is too much insulin can do damage o body orgins. going to see the indo doctor dec 13. just concerned at this time.

Actually, she is in intensive insulin therapy, with a rapid-acting insulin before each meal and a long-acting insulin that can control blood sugar for an entire day.

With this regimen, her blood sugar results are Ok!

If your mother were my patient, I'd suggest doing the injection of Lantus in the evening (eg. at 21:00 pm) and to control her fasting sugar for 2-3 days.

These fasting blood sugar results will be helpful to determine if we can decrease the doses of Lantus insulin.

At the same time, the 2h after meals sugar results are important to modify the doses of NovoLog insulin.

Taking too much insulin can cause hypoglycemia and this is a dangerous condition because prolonged hypoglycemia can lead to brain damage.

With the actual blood sugar results, your mother is not at risk for hypoglycemia.

However, it is necessary to monitor her blood sugar levels before and 2hour after each meal and to modify the insulin doses, accordingly to the results.

I have tried to cut down on lantus to 15 nutis in the morning and kept giving 6 unit of novaloc. sugers averaged 200 to 250. 20 units of lantus and 6 units of novaloc three times a day. sugers are at 165 to 185 in the morning. checked before meal in the evenings. sugers are at 151. thanks XXXXXXX

I apologize for the late reply.

Considering your mother's blood result I can say that with 20UI Lantus, her Diabetes is under control.

Decreasing the long-acting insulin to 15UI/24h actually increase her diabetes numbers. So, it is not enough.

If your mother were my patient I'd recommend this insulin regimen for her:

NovoLog 6UI, 5 min before each meal and Lantus 18UI at 22:00 pm
It is necessary to check her fasting blood sugar, before and 2hour after each meal results in the day after you start this regimen.

Record these numbers and if the result is OK then it will be not necessary to check her blood sugar several times each day.

Considering your mother's blood sugar levels with 15 UI and with 20UI Of Lantus,in my opinion ,your mother need 18UI Lantus.

Don't worry,your mother will not have hypoglicemic episodes with 18 UI Lantus,after switching to night because you mother actually is with 20UI and her Diabetes numbers are OK.

For the first time ,you try this new insulin regymen ,i suggest to do only 10UI Lantus at night because Lantus has twenty-four hour action.
So,if she take 20UI Ins Lantus at morning and than 18UI at night, she may have a hypoglicemic episode.

Anyway,this is only for the first time ,you switch Lantus at 22;00 pm.The fasting blood sugar will be helpful to understand her actual blood sugar situation.

I want to repeat the insulin regymen that i proposed for your mother.
6UI NovoLog Insulin ,injections 5 min before each meal.
10 UI Lantus at 8.00pm for the first day and 18 UI (the second day onwards) at 8:00 pm

It is necessary to check her blood sugars(before and 2 h after the meal) for 2-3 days after you start this new regymen to understand if she need further dosis adjustment.
