I passed carefully through your question and would explain that your symptoms could be related to myositis (inflammation of the muscles), considering the very high CPK levels. Sometimes psychosis is associated to myositis too.
Have you used any anti-psychotic drugs before?
Coming to this point, I would recommend performing muscle biopsy in order to examine your muscles and determine the possible cause underlying your situation. EMG should be done too.
Regarding the heart, you should know that changes in the ECG could be related to changes in the heart muscle (myocardium), which could be related to the rhabdomyolysis or any other muscle disorder (including myosistis or myopathy).
For this reason, I would recommend performing a cardiac ultrasound.
A cardiac MRI would be a more accurate tool to examine the myocardial tissues and examine the possible damages to the myocardial tissues.
An ambulatory 24-48 hours ECG would help examine your heart rhythm for a prolonged time, compared to a resting ECG.
