                                            Question : my MRI said I have a moderate sized effusion in my right knee . I've been compressing, Icing and have been prescribed anti inflammatory cream but it has been this way for 4 months . nothing is helping. what should I do ? 
                            
 
                                    I've also taken anti inflammatory pills aleeve for the past 4 weeks straight

I have gone through your query and reviewed attached images. Here is my advice.
As per your history and attached image, if swelling is since 4 months, then I think it should get further evaluated.
As per my knowledge, I think you may require fluid aspiration and it's analysis i e arthrocentesis and arthoscopy with damage repair after detailed medical examination and orthopedician opinion. As treatment mainly depends upon the underlying condition.
To reduce inflammation, You may also require steriod shot/injection after consultation.
Till then continue anti inflammatory pills and cream. Do compressing and icing. In case of fever, take tablet acetaminophen.
Avoid knee bending, long standing. Use support while walking.
