                                    Brief Answer:
I would suspect Gonorrhea
Detailed Answer:
Hello, I'm Dr. Branch,
Thanks for using 'Ask A Doctor' service.
In your situation, I would suspect Gonorrhea, given that Chlamydia and gonorrhea would be the most common organisms that would cause your symptoms in someone who is sexually active. If you had Chlamydia, that should have been treated with the Azithromycin and Doxycycline, but Gonorrhea may have been resistant to those antibiotics.
I would recommend getting tested to confirm gonorrhea, and make sure it isn't some other less common cause of infection. The best treatment would be a shot of ceftriaxone.
I hope that helps, please let me know if you have any questions, I would be glad to discuss it with you further.

                                    Brief Answer:
One shot of ceftriaxone
Detailed Answer:
Yes, one shot of ceftriaxone is the preferred treatment, usually with azithromycin and doxycycline, but I don't think you would need to take those again.
Pain in your hips and aching in the groin area is not really typical for a gonorrhea infection, unless it is from lymph node swelling in the groin area. I do think there is enough concern that you should get examined and tested, and then hopefully the cause will be clear.
Please let me know if you have any other questions, I would be glad to help any way I can.
