I've gone through your query in details...
I can understand the concerns for your son...

I've checked the attached photo, and it seemed like secondary to some kind of bump/external trauma (could've happened during semi-conscious sleeping also)...

He can start taking a mild analgesic i.e. ACETAMINOPHEN (eg. Tylenol), can be taken up to thrice daily after some food...apply a bit of local analgesic i.e. Diclofenac Spray over the area gently (don't massage) and observe how he stays afterwards...

Additionally, some cold compression (perhaps with a bag of frozen peas) twice to thrice daily would be helpful for the time being...
