Hi and thanks so much for this query.

I'd like to know if this is a new occurrence or has always been this way. If this is new, maybe there is some scarring that prevents easy distension of the foreskin when the penis is erect. If this has always been the case, then it is probably a longstanding issue.

The approach is different depending on your preferences. If you never want to deal with this and have no issues against circumcision, then I will recommend that. If circumcision is nothing you would consider, I suggest you get evaluated to identify areas of scarring that may need special attention to help with that.
