Generally psa is considered abnormal if it is more than 4. it is normal for the PSA to increase as you age because the prostate size gets bigger. however some doctors will consider PSA of more than 2.5 as abnormal. but be on the safe side I would recommend an MRI prostate examination to determine whether you need a prostate biopsy or not.

finding a hypoechoic nodule by an ultrasound is really inconclusive and doesn't mean anything. like I said earlier an MRI the better imaging modality for you.

further investigation is needed even if you have a normal PSA because it is rising from here 2015 result.
