Thanks for the query.
I have gone through the attached picture and old questions.

It looks like scar tissue after a biopsy. I think there are no signs of infection and nothing much to worry about.

It there is outpouchings or sprouting of tissues we have to suspect recurrence or hypertrophic scar.
Topical steroid injection like Triamcinolone injection will be useful.

It's advisable to consult a dermatologist and s/he will direct you accordingly.
