hello dear
thanks for your query at HCM
extensive work done in your child, lab data suggest that it could be iron defeincy anemia where child will have decreased hemoglobin , decreased mcv, increased RDW, ferretin decreased, total iron binding capacity increased, transferrin saturation decreasd.
your child has similar lab data with decreased transferrin saturation, increased TIBC, increased RDW both SD and CV. But hemoglobin and mcv are within normal range.
iron defeincy child will present with dull , not active, easy fatiguability, tired, pale child some may also have pica.
but your child looks quiet active and doing regular works
as far thalassemia is concerned it will have similar complaintss but lab data TIBC and RDW will be normal , simple test that will confirm diagnosis of thalasemia is hemoglobin electrophoresis.
you have done all test but forgot main test peripheral smear examination that will rule out most of the conditions. if possible get one peripheral smear test done.
regards
