What does it mean there are a few scattered T2 hyperintensities within the bilateral peri ventricular and sub cortical white matter, most comparable with minimal chronic small vessel ischemic changes... There is a punctate chronic focus of microhemorrhage within the left temporal lobe?

Your report says that due to blood vessel disease there were reduced blood supply to some parts since long that is why they have become ischemic. In left temporal lobe there is a small bleeding spot. You have to keep your BP and sugar level and lipid profile under control .
