For leg swelling Doppler ultrasound venous for lower limbs should be done to rule out any underlying vascular pathology. Also a fresh echocardiogram to look for size of VSD and whether it's interfering with normal cardiac functions to cause weakness or lethargy. Meanwhile if Amitriptyline is not getting along or causing discomfort should be stopped unless the cardiac work up etc is confirmed to be stable.
Hope I have answered your query. Let me know if I can assist you further.
Wishing you a speedy recovery.
