Hi I have had a really sore (R) knee for the past year or so, worsened 3 months ago I had an MRI done and it showed: a complex lobulated ganglion cyst in the medial anterior hoffa s fat pad, immediately anterior to the anterior horn of the lateral meniscus, it states that it is deep to the inferior aspect of the medial patellofemoral ligament / patellar retinaculum. it measures 29mm x 14mm axially and extends over a craniocaudal height of 29mm. it also said that there is moderately severe chondral thinning an chondral surface irregularity at the apex and deial facet. minor chondral surface irregularity of the lateral facet. and moderately severe central trochlear groove chondromalacia, with chondral clefting extending to the subchondral bone plate and subchondral cystic change. could you please explain what this means and if any treatment would be considered, regards Mila
Hi, Thanks for choosing healthcaremagic. As there's a complex ganglioncyst anterior to lateral meniscus there are chances of meniscal tear. And its almost 3 cms big so you need to get it treated. Chondral thinning and chondromalacia is not a significant problem as of now. These changes occur with age. You should undergo an arthroscopy and excision of the ganglion cyst alongwith repair of the meniscus if there are any tears. Along with that consult an orthopedician who will clinically assess the degree of chondromalacia. It might require repair if high grade and affecting daily routine life. Hope it sorts your problem and wishing you a speedy recovery. Regards, Dr. Pankaj Nagori Specialist Radiologist
