General & Family Physician 's  Response
Hi,
Holding of stool does not give passing blood in stool.
If stool is very hard and there is friction with rectum, might cause bleeding.
Possibility of having hemorrhoids should be ruled out.
If he is elderly then ultra sound of abdomen is advisable to rule out any space occupying lesion in colon.
Consult gastro-enterologist and get examined.
Ok and take care.
