General Surgeon 's  Response
Hi,

The causes of 4 year child unable to speak is usually no hearing.
- Since there is no hearing, he cannot learn the sound, hence cannot speak
- Adenoids are commonly enlarged at this age group
- Enlarged adenoids can alter the voice as the whole naso-pharyngeal structure helps in voice modulation

I would advise him the following:
- Consult an ENT surgeon for proper clinical evaluation and audiometry, MRI of brain and mastoids
- May be cochlear implant helps to improve hearing and then a proper speech therapy helps to speak normally

Take care. Hope I have answered your question. Let me know if I can assist you further.
