Dear sir/madam, he is having both renal and cardiac problem. Both system are interlinked. Low cardiac output worsens renal function and worsened renal parameters can put extra workload on heart which in turn decreases pumping capacity. Ideally he has to managed medically. Exercise wont improve his Cardiac (heart) function. By your description he looks to be in late stage ( advanced disease ) of both kidney and heart. just follow instructions as per your doctors advice. Any intervention will carry grave risk to the patinet.
