Cough, Cold, Mild Wheezing, Excessive Sneezing, Throat Congestion, Occurs In Cold Climate. Cure?

Hi Dr., I generally suffer from cold and cough during rainy/winter season accompanied by mild wheezing and extreme sneezing . I however try to keep my self warm and also avoid cold beverages. But the wheezing seems to come back every winter and causes a lot of suffocation especially during the night and early morning. Can you please help me?

Hi,
Symptoms described you are pretty consistent with asthma orallergic rhinitis. Asthma is charaacterised by nocturnal and seasonal variation which leads to frequent episodes of cough and cold particularly in winter/ rainy season.Keeping yourself warm and avoiding beverages will help a little bit in avoiding infection but it will not keep asthma under control. However I cannot make a definite diagnosis based on your history alone You should undergo atleast pulmonary function test.If diagnosis of asthma is made, then you need to take salmeterol-fluticasone B.D or formoterol-budesonide B.D along with montelukast-levocetrizine once daily. In case of allergic rhinitis, you need to take fluticasone-azelastine nasal spray twice daily. As you described nocturnal awakening along with suffocation means your asthma is uncontrolled. You should better consult a pulmonologist for a detailed clinical evaluation.

Hello,
Your symptoms of constant cough, cold, mild wheezing, excessive sneezing, throat congestion in the cold is likely aggravated by house dust mite allergy apart from the occasional viral infections that can trigger asthma.

That your symptoms worsen in the winter is likely due to house dust mite allergy as these very tiny insects multiply rapidly when central heating is on. I am presuming you are in the western part of the world.
Blood tests such as specific IgE against house dust mite, mold allergens are important that can delineate your allergies. Skin prick tests can also be done if blood tests are not available n your area.
A combination of fexofenadine in the morning, nasonex nasal spray 2 sprays/nostril twice daily to start with (then once daily), montelukast 10mg at night plus/minus asthalin inhalers will be the optimum medical management pre-winter 1 month and throughout winter.
If you fail medical therapy & proven dust mite allergy, immunotherapy over 3 years is the final treatment.
Thanks.

Hello.
Thanks for posting your query.

You might be suffering from Asthma. Asthma is a problem in the airways due to multiple factors. The airways in a person with asthma are very sensitive and react to many things, which are referred to as "triggers."

Common asthma triggers include:
Colds, flu, sinus infections
Cold air, changes in temperature
Tobacco smoke and air pollution
Substances that cause allergic reactions in the lungs, including dust mites, pollens, pets, mold spores, foods, and cockroaches
Dust or items causing dust
Strong odors from chemical products
Anxiety, and activities like crying, yelling, or laughing hard
Aspirin, ibuprofen, and beta blocker drugs used to treat conditions including high blood pressure, migraines, or glaucoma.

By avoiding asthma triggers, taking medication, and carefully monitoring daily asthma symptoms, asthma attacks can be avoided or at least limited. Proper use of medication is the basis of good asthma control. Drug used to treat asthma include bronchodilators, anti-inflammatories.

Wishing you good health.
Regards.

Internal Medicine Specialist Dr. Akhilesh Dubey's  Response
Hi,
Welcome to HCM
Your problem is quite common one and seasonal allergic wheezing indicates a Bronchial asthma due to naso bronchial allergy.
Beside keeping warm and avoiding cold beverages you have to see a Chest Physician and a detail examination with proper history .
Then a Pulmonary Function test with effect of bronchodialator will confirm the diagnosis and quick relief medicine may be given in inhalation form.
Allergic status can be checked with serum IgE level an allergy testing.
If you take anti allergic and bronchodialator and if required steroid in inhalation or oral form will help you a lot antibiotic if required.
If particular allergy is there immunotherapy may give you long lasting relief.
Once you improve you can take up yoga and Pranayam.
Take care
Best of luck.
