Could A Delayed Period Be Due To A Pregnancy?

My last menstrual cycle was on 15th Feb and during that time i was away from my husband.I flew down to Australia on 24th Feb and since then me and my husband have been having unprotected sex every alternate day but my husband never used to cum inside. My concern is that i have not yet had my periods this month.my periods are always regular.I am suffering from thyroid since past 13 years and have been taking regular medication for it since then.I just want to know if i could be pregnant? i took home pregnancy tests twice within last 5 days and both of them turned out to be negative. What should i do now?

Hallow Dear,

I presume your regularity of the menstrual period is around 28-30 days.

Since you had last period on 15 February, your egg release would be somewhere around 1 March to 3 March. Considering the 24 hours' life of egg and that sperms are active for 72 hours, your fertile period should be from 26 February upto 7 March. You did have regular sexual relations with your husband during that period.

Though he has not ejaculated inside you, there is always a possibility of pre-ejaculation secretions (precum) getting released in your vagina. These secretions get expelled out without sensations. Precum does contain sufficient amount of sperms which can cause pregnancy.

If a woman having regular menstrual cycles in a childbearing age group with regular sexual relations misses her period, the first differential diagnosis is pregnancy, unless and until proved otherwise.

Though your pregnancy tests reported negative, the question is when were they done. These tests are reliable after about a week of the missed period; done earlier, they may report false negative. So you may repeat pregnancy test on the overnight first morning urine sample after one week of missed period. Alternatively, you may opt for Beta hCG test on the blood. Values above 25 mIU/ml are diagnostic of pregnancy while in non-pregnant state, the values are under 5 mIU/ml.

Moreover, you are a chronic patient of Thyroid dysfunction which also may cause irregularity in menstrual periods if uncontrolled.

If with the tests, you are proved to be non-pregnant, you may take some progestational preparation like Deviry for 5 days and then within 4-7 days, you should get withdrawal bleeding. However, then please get your Thyroid function tests (T3, T4, TSH) done to assess Thyroid function.

I hope this clarifies your doubts.
