Dear person, smoking is always dangerous and written on it's package. Smoking can cause reduce functioning of lung. Emphysematous changes of lungs means reduce elasticity of your lungs which may be responsible for tightness in chest.Because your normal respiratory muscle not able to provide sufficient oxygen due to bad lungs other muscle from neck and shoulders which attached to chest, try to push the lung for more oxygen. So you are getting pain on these sites. better to consult deaddcion specialist and quit smoking as early as possible. All the best!
