Diabetic. Taking Triglycomet, Glibenclamide, Metformin And Pioglitazone. FBS Is 280. How To Control It?

hello sir i am diabetic type 2 and iam taking Triglycomet with combination Glibenclamide 5mg, Metformin
HCI SR 500mg, Pioglitazone 15mg twice a day but this drug is banned and my FBS 280 and PP is 330 around .. please suggest me medicine to control this percentage my weight is 40 kg and no problem of heart diseases but sometimes i have digestion problem

Hi Thanks for using HCM.

I advice you to start Insulin. FBS more than 200 and uncontrolled diabetes even with the above drugs are the indication to start insulin. Consult your treating doctor to start and adjust the dosage. You should also do life style modifications along with medicines.

hello,
from your letter , it is not clear how long you were on this drugs, what is your present glycemic status, your age ...

I would suggest the following plan of action
1. get your Diabetic tests done, complete with HbA1C, BUN,eGFR and lipids
2. according to your results, there might be a need for reviewing your treatment regimen, you may consider starting analogue insulin once a day along with DPP-IV inhibitors and Metformin for some days then repeat your tests and titrate
3. check for complications and do consult a Nephrologist, cardiologist and ophthalmologist soon
4. ask your doctor to advice on self monitoring of blood glucose and specific timings
5. pl look into lifestyle modifications with a proper planned diet and exercise regimen

The ideal option will be insulin...
There are several classes of drugs / and several drugs in each lass...
Discuss with your doctor (Diabetologist)

Initially , take drugs individually / switch to combination product, when stabilised
Metformin may be stepped up – 500 mg to 2 or 3 times / even more!
Gliabenclamide may be stepped up – add half before supper / even more / there are newer and safer options...
Voglibose may be added..
And so on...
Ask your doctor
