Do The Lymph Nodes Float In Between The Liver And Kidneys?
A recent CT scan of my 35 yr old daughter's abdomen showed lymph nodes floating between her liver and kidneys, which the doctor and the radiologist were totally flummoxed by as per them this just doesn't happen. She also has moderately elevated liver enzymes but her liver and other organs appear to look just fine. She has pain in her right upper abdomen and it hurts to take a deep breath...she also has plueral effusion of her right lung. She will be having 2 more CT scans of her chest and pelvis next week. What on earth could be going on?

Emergency Medicine Specialist, Dr. Tim Evans's Response
There is clearly a problem to account for the fluid in the lung, a pleural effusion, her abnormal liver function tests, a tender liver and enlarged lymph nodes in the abdomen. It seems that the CT has not been able to provide a definitive diagnosis. She will require further tests in order to identify the problem. She will likely undergo an MRI scan and multiple blood tests in the first instance. She may need a sample of fluid taking from her lung, called a pleural tap, in order to determine whether the underlying problem may be inflammatory/infective or other causes. She may also need a biopsy, or a sample of tissue taking from her lymph nodes or her liver. It may be a primary lung problem or a primary abdominal problem. Her physician will likely get further opinions from other specialist doctors, such as a hepatologist (liver specialist) and a respiratory/chest physician. She is clearly at the start of her investigative process and you will understandably have concerns and worries. Keep doing your own reading and research and write down any questions you might have. Do not be afraid to take out your list of questions and put them to your daughter's physician at her next appointment.
