Does MRI Findings Of Disc Herniation Require Surgery?

I have hernation in l 5 and s1 with 9mm disc and nerve compression frm rt to left. I ve tingling feel on nerve rt leg with pain on lower calf muscle,hind string, rt buttocks specially when i sit on office chair, and when i drive a car .l get relieve with in the morning. It started four weeks oback.after i fell down while playing football, it is same
old case which occurred in 2004 and recovered . After seeing mri report doc told me it require surgery. I m 42 years old .pl advice me.
