Does ST Johns Wort Decrease The Effectiveness Of Nexplanon Implant?

OK this one may sound weird, I am on nexplanon any my husband does not know if he wants any more children (we have one that is 7) but I know I do. I have had the fever for a while and it is only getting stronger. He states if it happens then it happens. Ok so I started taking ST Johns Wort because I know it can reduce the potency of the implant. I take between 1800 to 2700mg per day (900mg two to three times a day with meal). What is the likely hood of actually getting pregnant. I am going to talk with my DR about getting the implant removed but he does not think my insc will approve it since it has not been in for more then 3 years. I know if you do get preg they are mandated to remove it. I will be 33 this year and I would to enjoy my child and not worry about all the potential risk factors that come with being pregnant after 35. any advice that may help.

General & Family Physician 's  Response
Hi,

St John's wort is a plant that can be used as a treatment for depression. On the other hand, Nexplanon implant contains etonogestrel, a female natural hormone called progesterone that is used in the prevention of unwanted pregnancies from unprotected sexual intercourses for up to 3 years.

St John's wort can interact with hormonal contraceptives like Implanon implant, reducing the effectiveness by causing an induction of ethinyl estradiol norethindrone metabolism consistent with elevated CYP3A activity and increased the risk of unplanned pregnancy. According to some clinical studies, treatment with St John's wort is associated with a significant 13-15% reduction in the dose exposure from the contraceptives like Nexplanon implant.

Take care. Hope I have answered your question. Let me know if I can assist you further.
