Hello,
If fetal cardiac activity is not seen in a trans-vaginal sonogram done at 6 weeks of pregnancy, it is abnormal. Yet, as benefit of doubt, a repeat sonogram is to be done after another week since the cardiac activity may be missed in the earlier sonogram. Simultaneously, serial titres of serum beta-hCG can be done to assess the progress of the pregnancy. Doubling titres every 2-3 days is suggestive of good progress. If after everything, the pregnancy is missed, spontaneous expulsion can happen in a week or two, but, the ideal way of management is to induce a medical abortion under surveillance. Hope this helps.
