x ray. 
Laryngectomy patients are at increased risk of aspiration. 
And this aspiration can lead to 
aspiration pneumonia. You are having cough, mucus expectoration and 
hemoptysis (blood in sputum) with pain in chest. 
All are suggestive aspiration pneumonia. 
So to rule out this chest x ray is must. 
So consult doctor and get done chest x ray. 
You may require additional antibiotic.
