Does Heavy Bleeding Require Immediate Medical Attention?

Hello I am bleeding alot and don't know if I should go to the er. Two nights ago woke up in tears from really bad excruciating pain in my back and stomach and whole private area I had too have help to the bathroom it felt like I had too push but nothing came out, then woke up in the next day and was bleeding alot since then I've been wearing pads and tampons and it's still seeking through I'm changing atleast every hour , what should I do ?

Consult gynecologist as early as possible and undergo complete clinical evaluation and USG scan to rule out cause
