Does Low Thyroid Hormone Affects Fetal Development And Is Thyroxin Good Enough?

Hi my wife is 4month pregnant..... Her TSH level shows 7.46 uu/ml ....

At 26/06/2013 Her TSH level shows 9.42 uu/ml and the she take thyroxin 100mg as per Dr says.

At 26/09/2013 Her TSH level shows 0.03 uu/ml and as her Dr’s order she changes the power and takes thyroxin 50mg.

At 07/12/2013 we test TSH and found the Level is 0.11 uu/ml and as per Dr. says the in that time she is 30days pregnant. Dr. advice her to stop the thyroxin.

At 30/01/2014 we test the TSH and found TSH level is 7.46uu/ml. immediately we consult our Dr and he advice her to start / take thyroxin 50mg.

We are big confused. My question is (1) in this condition is thyroxin 50mg good enough for her? (2) Does this level (7.5) have any effect on foetal development... is any possibility to born abnormal baby?

Endocrinologist 's  Response
This is subclinical hypothyroidism in pregnancy. The upper limit of TSH beyond which treatment with levothyroxine should be started is 2.5 mIU per litre in the first trimester and 3 mIU/L in the subsequent trimesters. The most appropriate dose for you will be 50 or 75 mcg daily and to decide on dose, you should consider checking the TSH more frequently like every month. This level will not have any major effects on the foetus even though subtle difference in IQ is shown in some studies. Most importantly treatment is given to avoid abortion, preterm labour and precclamptic toxemia.
