Does Nutritional Deficiency Cause Poor Weight And Height Gain In A Toddler?

My baby s weight percentile dropped from 50 at 5 months of age to 3 at 1 year. Her height has dropped from 90 percentile at 8 months to 25 percentile at 1 year. I m very worried. We started her on solids from 5 months. I m a working mom and I breastfeed her when I return home. I m very worried if she is suffering from any nutritional problem.
