Does Severe Pain In Foot After A Fall Suggest Sprain?

Last night I stepped in a hole with my right foot that was Atleast 8 inches deep and I fell. My right foot is killing me. When I m sitting and try to pull flex my foot back toward my shin only my big toe will come back and my other toes kind or curl down. I now also have a good size indention in my ankle/top of my foot when I flex my foot toward my shin. I m also have pain in my knee and hip now. My knee and foot is very weak and I don t feel stable while standing or walking. I m having a little swelling in the top and outside of my foot but nothing major and no bruising. What could I have done to my foot. Is it just a sprain or could it be broke?

General Surgeon 's  Response
Hi,

Trapping of one foot in a deep hole caused the symptoms which I noted as you have mentioned. The possibilities are:
- Sprain that is tear of the collateral ligaments of the joint is the commonest cause of pain and other symptoms
- Fracture of metatarsal and fibula is know and can cause pain, swelling, inability to move the joints as you have mentioned

X-ray of the foot and ankle with lower tibia and fibula should be done in 2 views. X-ray of ankle may need special angles. MRI or CT if required. Take an anti-inflammatory along with painkiller properties. Further management like slab, cast as per the reports. Till then give complete rest and apply elastocrepe bandage to stabilize the joint. Consult an orthopedic surgeon for proper clinical evaluation, correlation with reports and get treated as per the cause found.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
