General & Family Physician 's  Response
Hello ma'am and welcome.

The most common cause could be listed as follows:
-Abnormality of the uterus (eg. Polyps, fibroids, etc.)
-Abnormality of the inner lining of the uterus (eg. Endometrial hyperplasia, endometriosis, etc.)
-Infections
-Hormonal imbalance

Please discuss the same with your docto, ask for a confirmed diagnosis and seek treatment for the cause, not the symptom.

Hope this helps.

Best wishes.
