Have A Headache. Feel Heavy Pressure On The Head. Had Insomnia, Seem At The Time Bipolar. Have Visions Of A Man. Treatment?

I have a headache heavy pressure on my head my eye sockets are sore ,a voice in my head is real and makes me feel sick sometimes He has been my "friend" for fourteen years, ok the white coats were called in long ago when I revealed this fact, I was hospitalized ,I knew it was insomnia that made me seem at the time bipolar , .I had to retract my story to get back home to my family. I am a highly functional human being ,just in pain and exhausted by the constant voice, he is funny most of the time and I laugh a lot but sometimes he is frightening, No he can never tell me to do things. He just asks me questions all day long, I sometimes have two conversations going on simultaneously, with him and a visible person. He is a real person,. I often beg him to go away .
I have met him, but he does not want to know me in the real world, just in telepathy.
