Stress , worry will increase your problem.
You main problem is weight. Your BMI as per your age & weight is 37.9, which is high. If you reduce your weight, your rest of problems will be easy to handle.
Hypothyroidism also can be cause of weight gain.
Join a good gym near your house. You will meet new friends that will keep you motivated & encouraged. Do any form of exercise which you enjoy doing. This is reduce your stress & also make you feel happy.
Consult your doctor & adjust the right dose of medicines to keep your sugar levels & thyroid levels in control.
Eat high fiber diet, Reduce portion size of your food intake, Eat small frequent meals at regular time, avoid fasting for prolonged time.
Drink more water. Reduce tea, coffee intake. Avoid fried food, fast food items and
cold drinks.
Be consistent in your efforts & within few months you will see the results.
