Have Renal Failure. High Blood Pressure. ER Then ICU. Have Hemo Treatments. Effect Of Endocrine, Pituitary?

yes please. a bit of history first. Female 44 end stage renal failure. having reoccurring monthly episodes of critically high blood pressure during the period before my period. 13 epidsodes thus far, 250 over 130 to 140 or so. ER then ICU usually. Nicardipian only thing that helps via IV. So my questions, should I lessen my hemo treatments during that time because they are filtering out too many of the helpful hormones being released by my body at that time of the month to help me naturally combat these type of reactions. I've been reading up on endocrine, pituitary, and the effect they have on woman via the blood during that time of the month. what do you think?

Hello

I am Dr Abraham. I come to know that you are having episodes of high Blood Pressure. In end stage Renal Disease it could be due to several reasons
1. pre existing BP which has become worse - you need to review your medications and consult with your nephrologist for a tighter BP control.
2. Dialysis itself can cause Hypertension as you are connected to a high pressure system to filter the impurities present in blood. But your kidneys are unable to do this job themselves. Consideration for Renal Transplant has to be done as sudden surges can be a threat to life and can become dangerous.
3. Do you take any pain medications? Pain killers can cause failure or aggravate the failure more.
4. Are you on any erythropoeitin injections which are given to increase your hemoglobin? They can cause Hypertension if hemoglobin goes above 13 mg/dl.
5. have you got any breathlessness, swelling of your feet, cough? Problems with heart can also cause your BP to shoot up? A chest Xray and echo may detect these.
6. If you have diabetes or high blood cholesterol their levels need to be checked. They need to be controlled as poor control of blood cholesterol nd glucose will also increase your BP.

Thanks

Dr Abraham
