Hi,
Welcome to HCM,
You are having Diabetes Type II, and taking Januvia, but you must understand that we need to take care of diet and check blood sugar in between also, any fruit juice contains refined sugar which should not be used or should be taken in less quantity.
Now when your blood sugar is high, you should get your urine sugar and ketones to see if there is ketoacidosis.
If ketoacidosis is there you need to get hospitalize and treatment with insulin and proper fluids IV.
If there is no ketoacidosis then your treatment may be planned with combination of antidiabetic medicines like Metformin and sulfonylureas with or with out Insulin and taking care of your diet avoiding refined sugars and taking small and 3 or 4 meals.
In diabetes treatment can not be according to our choice.
Please take care

Internal Medicine Specialist Dr. Mayank Bhargava's  Response
Hi,
Thanks for posting your query.
Diabetes is a multi organ disease with involvement of almost all organs of body due to its complications.
Blood sugar both fasting and postprandial must be in controlled range.
Diabetes is due to progressive loss or destruction of beta cells in endocrinal part of pancreas.
When diabetes has been diagnosed, less than 20% beta cells remains in pancreas and progressive destruction of beta cells is still continuing.
Rise in blood sugar is due to progressive loss of beta cells.
You should avoid soda and you can take orange juice without adding sugar.
You should consult with internal medicine specialist/ endocrinologist and should go for thorough check up.
You should go for blood sugar fasting and postprandial, glycosylated hemoglobin (HbA1c), serum C peptide and lipid profile.
Whatever we take orally has some amount of carbohydrates. So sugar level should not be considered deranged by edibles.
Take proper treatment from your local doctor.
Take care,
Dr. Mayank Bhargava

Hello,
Fruit juices and colas have tremendous amount of sugar in them.
While a 12oz orange juice has 36 gms of sugar,12 oz of coca cola has 40 gms of sugar.
If you keep on drinking these sugary drinks, your blood sugar levels would always tend to remain out of range and high.
You must make it sure that you stop consuming them immediately and make all efforts to keep your sugar levels in normal range if you want to avoid future micro as well macro vascular complications.
Uncontrolled diabetes damages your all most all the organs in your body.
Along with dietary restrictions, start doing regular aerobic exercises daily for atleast 45 minutes a day.
Reduce your weight and keep taking your medications regularly.
Thanks
