Having White Patches, Fungal Infection

Hi, My wife has developed some white spots and hypopigmented patches in her underarm and breast area, we went to a dermotologist, but doctor was confused about whether it is vitiligo or fungal infection, doctor priscribed fluconazole tablet with mometazone and terbinafine ointment. she applies ointment regularly but there is no improvement, what it could be ? Her mother has vitiligo covering her full body.

If her mother is having Vitiligo, then chances are that she will also have it.. did she not tell the doctor that her mother has it? get another opinion if in doubt and inform about family history of vitiligo.
