Heart Palpitations, High BP, Inducible Ischemia. Underwent TMT, Done ECG. What Can We Expect?
I am a 40 yr old male. Recently i underwent a TMT test on my wife's insistence. It showed Mildly Positive for Inducible Ischemia at Moderate workload. Sir, could you tell me the meaning. The result is as follows
Mr ____ exercised for 10 mins and 50 secs on Bruce Protocol. The resting heart rate was 95 beats / min and resting BP was 140/90mmHg. Baseline ECG did not show any significant ST-T changes. He achieved a maximum heart rate of 160 beats/min which was 88% of age predicted maximum heart rate. The blood pressure response was normal (Maximum BP achieved 170/100mmHg). He achieved a workload of 12.44 Mets. The reason for termination of the test was achieved THR. There was no Angina. Significant ST-T changes were noted at peak exercise in lead III 1.1mm(H). These changes recovered within one minute of recovery period. No significant arrhythmia was seen.
Result
1. Treadmill test is Mildly Positive for Inducible Ischemia at Moderate workload.
2. Normal BP response.
3. Good exercise tolerance.

The finding is limited to a single lead...it is an indication for further work up – because this may be false positive... The next step is TMT with thallium isotope. It is the ideal non-invasive way to evaluate ischemia / to assess the PHYSIOLOGY (function) – to see whether the blood arriving at the heart muscle.
If there is a suggestion, the next step is to see the ANATOMY (structure) – CT angio is non-invasive study for the anatomy. If positive, you may need catheterisation...
