Hello Doctor, When I Was 16 Years Old I Developed

Hello Doctor,

When I was 16 years old I developed a habit of like seeing any women on tv or in area and then going to bedroom and pressing penis with both my hands and imaging I am having sex with that lady and after 5-10 mins I go to toilet and white fluid comes from my penis and then urine comes.I am doing this daily from last 10 Years daily.I have following questions:
What problem.I am facing?
What are the problems caused to this in future and what are the solutions to this
Also when I was 16, my penis cap was opening whole but now after 10 years I tried penis cap is not opening.When I try,I get a pain.Is it caused due to this habit?
Am I looking my sperm count and would this affect my sex life and would I have problems in having kids in future as I am planning to get married in next 4-5 years.
Would my urine STOP if continue this habit?

Please advise me a good solution as I want to solve this issue and avoid any problems in future?
