Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query about your husband and here are my thoughts.
COPD is chronic
obstructive pulmonary disease and it is necessary to know the stage by doing
CT scan of the chest which decided the treatment.
Stiolto respimat is a
bronchodilator and once inhaled can cause the problems of coughing.
I think he is doing this incorrectly considering the the way the coughing is presented the moment the spray hits his back of throat.
I would advise him to get a correct way to inhale by consulting a Registered Nurse or a Doctor wherein he has to coordinate inhalation that is deep breathing in and pushing the inhaler at the same time. This may take time but once mastered the coughing problem will go away.
In some cases it may be needed to change over to another brand if there is a specific sensitivity to the one he is inhaling.
Also needed is a proper evaluation by a Pumlmonologist, investigations like PFT that is pulmonary function tests, CT scan, culture and sensitivity of the sputum and so on to get an exact idea of the stage and thus plan management as needed.
Chest physiotherapy is part of the management.
Hope I have answered your query. Let me know if I can assist you further.

Dr. T Chandrakant
