I can certainly understand your concern I have worked through your query in detail. there is nothing to worry about the brite rash in the armpit, as this could be due to the common allergic side effect of mellaril, ie thioridazine( typical antipsychotic), you can start him on cetrizine to get rid of that rash you have mentioned that he has a paradoxical effect due to sedatives, you can start him on benzodiazepines like clonazepam in the night with initial minimum doses to combat with his sleep, and start on some SSSRI s like Escitalopram in the morning if he have any symptoms of anxiety Please check with your physician if he shares my view Post your further queries if any.
