How Can Ankle Swelling With Fluid Causing Pain And Stiffness Be Treated?

Hi I have a swollen ankle which develops a build up of fluid throughout the day - and looks like an onion - but slims down at night during sleep. It can be very painful and stiff. This is the only joint affected and numerous doctor's appointments have produced no diagnosis or treatment. Could this be gout?
